Location: Beatty High school GYM/Multipurpose room
!! Please, park by the football field and enter the gym through the locker room doors. All other entrances and areas are off limits.

Time: 10 am
Cost: $20 for physical, free for Medicaid, and a $7 concussion test fee (not required for middle school). If you have never had a concussion test, you will be required to take one.
-Physical forms will be on site but must have parent signature. --Physical forms can also be picked up prior to physical date at BHS or BEMS office.
