Beatty COMMUNITY IMMUNIZATION EVENT

((( Anyone interested can come to the school to get a printed copy of the packet. )))

Thank you for your interest in this event.  We hope to be able to immunize as many children as possible so there is no delay when school begins.  To ensure that can happen, we need your assistance by completing and returning the following:

Please complete, sign, and date the forms in the packet.  In addition, please send a copy of the following:

• Medical Insurance Card (if you have any insurance besides NV Medicaid or Blue Cross Blue Shield, please call office to see if we can accommodate)
• Proof of household income.  See the income verification form for examples of types of proof of household income documentation.  We are grant funded and are required to have proof of household income for all clients. 
• If child was vaccinated outside of Nevada, bring in the vaccine record as soon as possible but at least 1 week prior to the vaccine event.
• If we do not receive all required documents prior to the vaccine event, we will be unable to vaccinate your child on that date.  We will reschedule your child for a different date and time.
• You may choose to complete the forms in the office electronically, but required documents must still be brought in at that time.
• If you are not the parent but are the guardian, we will need a copy of the guardianship documents.

Please return to the Pahrump Community Health Nurse office.  If you are unable to return documents to the Pahrump office, please call so that we can be sure your child’s information is in our system prior to the event.  If you have any questions, please feel free to contact us at (775) 751-7070 between 7:30am and 5:00pm Mon-Thurs.
